We’re excited to kick off the second annual Seamless Care Awards — a program created to celebrate the incredible achievements of our healthcare partners. These awards spotlight the exceptional work our partners do to improve patient care through digital innovation, change management, and research.

This year, we’ve been blown away by the level of impact across our partner network. From scaling digital care journeys to achieving meaningful improvements in patient outcomes, each finalist represents a commitment to transforming the healthcare experience.

After a thorough review of engagement data, implementation milestones, outcome metrics, and customer stories, our Customer Success team has selected the top finalists in each category:
