↓ 52% Readmissions

Cardiac Surgery:

↓ 47% Emergency Department visits
↓ 16.7% Average Length of Stay

Patient Feedback:

  • 98% felt more confident before surgery
  • 95% felt more confident at home after surgery
  • 89% avoided 1+ phone calls to the hospital

Following this success, Horizon has expanded SeamlessMD beyond surgical care to other areas, including Congestive Heart Failure (CHF), maternity care, hepatopancreatobiliary (HPB), and more – ensuring patients across multiple care journeys benefit from the same proactive support.
