  • Patients receive daily, evidence-based education about COPD – for example, what the condition is, what causes it, and how to recognize flare-ups
  • Clear expectations of the “red, yellow, green” COPD zones help patients know when to seek help
  • Step-by-step resources on inhalers, oxygen therapy, and devices like pulse oximeters and peak flow meters
  • Embedded videos and illustrations make learning simple and practical

Personalized Support & Daily To-Dos

  • Each patient receives a tailored checklist of reminders, such as:
    • Taking daily medicines and inhalers
    • Using oxygen therapy
    • Tracking breathing at home
    • Completing breath training exercises
    • Avoiding triggers and practicing smoking cessation
  • “Learn How/Why” links connect directly to an education library for deeper understanding and motivation

Symptom Monitoring & Flare-Up Detection

  • Daily “COPD Zone Check” walks patients through key symptoms like breathing changes, swelling, coughing, wheezing, energy, and sleep
  • The symptom gives personalized recommendations – from self-care tips to immediate prompts to contact their healthcare team
  • Care teams can get alerts and dashboards to intervene sooner for patients showing signs of exacerbation

Whole-Person Care & Resources

  • Patients can track quality-of-life metrics such as sleep, exercise, and mood.
  • The program can assess social needs like access to food, housing, or transportation and guide patients to community resources.
  • Validated tools like PROMIS-10 and other COPD-specific measures can be integrated to track outcomes.
