Celebrating Horizon Health Network’s Success with SeamlessMD for Heart Failure Care

We’re proud to celebrate the outstanding work Horizon Health Network is doing to support heart failure patients using the SeamlessMD platform. Since launching the technology in 2023, Horizon has expanded its use of SeamlessMD from cardiac surgery to hip and knee replacement – and most recently, for heart failure patients – with additional care plan implementations underway.

A powerful example comes from 72-year-old Robert Conrad, who shared his experience at a recent Horizon board meeting. After being discharged from Saint John Regional Hospital with congestive heart failure, Conrad began experiencing extreme dizziness and fatigue – a common side effect of heart failure medications. Using SeamlessMD’s real-time symptom monitoring and personalized feedback, he was prompted to contact his care team. Within three hours, his medications were successfully adjusted.

As Conrad shares: “I’ve had a really good experience with the app. Since then, my blood pressure has been stabilized, my heart rate is stabilized, my breathing is much better than it has been.”

To date, several dozen heart failure patients have been enrolled in the SeamlessMD program through Horizon. The app provides daily reminders, symptom tracking, educational content, including videos to help patients follow their care plans and stay engaged in their recovery.
