Southlake Regional Health Centre Implements SeamlessMD Digital Care Journey Platform

Toronto, Ontario – June 13, 2023 - SeamlessMD, is pleased to announce that it has completed a successful implementation of its Digital Care Journey platform at Southlake Regional Health Centre. With this deployment, Southlake, recently named in the Top 10 hospitals in Ontario and Top 25 hospitals in Canada, joins other leading healthcare organizations in Ontario leveraging a digital health platform to automate care delivery including interactive patient education while enabling clinical teams to provide more proactive, efficient care for their patients before, during and after hospitalization.

Part of the Southlake Community Ontario Health Team that serves the northern York Region and southern Simcoe County, Southlake cares for some of the fastest growing and aging communities in Ontario. SeamlessMD’s patient-centric technology offers accessible education and enhances Southlake’s surgical remote patient monitoring strategy, starting with knee replacement patients. 

“With SeamlessMD, we have a digital technology partner that has proven to deliver improved outcomes with other hospitals across Ontario, and the platform's educational and enhanced remote monitoring capabilities will ultimately be impactful in improving our quality metrics, such as reducing emergency department visits and readmission rates​​,” adds Dr. Patrick Gamble, Division Head, Orthopedic Surgery at Southlake Regional Health Centre.

With SeamlessMD, care teams can virtually guide patients from pre-surgical preparation through post-surgical recovery by delivering consistent, evidence-based education, progress-tracking, and symptom monitoring using their own devices (e.g. smartphone, tablet computer). While recovering at home post-surgery, patients can self-report data such as pain scores, symptoms, range-of-motion, and share photos of their wounds through the platform remotely, enabling them to actively participate in their own care. 

Meanwhile, providers can monitor patients on dashboards and receive alerts for serious concerns, allowing for faster intervention and personalized recommendation as needed. The platform is fully compatible with Southlake’s patient information systems.

As one Southlake patient shared, “I found this program very helpful. It provided much needed information about the upcoming operation, check-in procedure, what to expect after the operation and follow up appointments. I liked that the one query I did have was addressed by the team via a phone call to myself. It was nice to know that you could reach out when you were overly anxious or had a concern. Though I did not check in every day, I knew the importance of doing my exercises and self care. I thought the program was great.”
