Toronto, Canada – March 28, 2023 – SeamlessMD, a Digital Care Journey platform that has been widely adopted across North America by leading hospitals and health systems to enhance the delivery of digital care, announces its successful deployment of a remote patient monitoring system for Oak Valley Health. SeamlessMD’s patient-centric technology will help Oak Valley Health, one of Ontario’s leading community health care organizations providing high quality, patient-centred care to more than 437,000 patients each year, deliver an enhanced digital experience that guides patients step-by-step through their surgical journey. At this time, Oak Valley Health is rolling out SeamlessMD to digitally support patients undergoing hip, knee, shoulder, and spine surgeries.
With SeamlessMD, patients at Oak Valley Health are guided pre- and post-surgery via evidence-based education, reminders, videos, and progress tracking surveys and they can self-report data, all through their own device (e.g. smartphone, tablet, computer). Oak Valley Health’s teams also benefit from being able to remotely monitor patients, view patient data such as care journey compliance, survey results, and opioid use, as well as access alerts, allowing for faster intervention for patients at risk and the delivery of better care. The platform is fully compatible with Oak Valley Health’s patient information systems.
