SeamlessMD and Xealth partner to Streamline Adoption of Digital Care Journeys

Toronto, ON – July 31, 2025 – Today, SeamlessMD is excited to announce a new integration partnership with Xealth, a leading digital health platform, to make it even easier for health systems using Xealth to enroll patients into SeamlessMD’s digital care journeys and monitor the success of their programs.

With this new integration, health systems using Xealth can now automate patient enrollment into SeamlessMD’s digital care journeys based on key events such as a scheduled surgery, outpatient appointments or hospital discharge.

Once a triggering event occurs, patients are automatically enrolled into SeamlessMD through Xealth, without the need for manual intervention. This ensures timely delivery of personalized education, reminders and symptom monitoring - across surgery, oncology, women’s health, chronic care and more.

Additionally, the SeamlessMD <> Xealth integration supports:

• Automatic receipt of surgery, appointment and discharge updates via Xealth, ensuring care plans are updated in real time
• Ability to view patient enrollment status directly in the EHR via Xealth, so care teams can easily confirm whether a patient has been enrolled in SeamlessMD
• Continued access to any SeamlessMD’s EHR-integrated dashboards and tools, enabling providers to monitor alerts, track engagement, and view patient-reported data within their existing workflow
