Care teams work from a patient roster with configurable columns — enrollment date, procedure, days since surgery, last survey completion, outstanding alerts, PROM scores, risk level — and can drill into an individual patient view showing a flowsheet-style timeline of all patient-reported data with out-of-range values highlighted. Where SeamlessMD is integrated via SMART on FHIR, these dashboards sit inside the Epic or Oracle Health patient chart. Teams typically work by exception, reviewing a prioritized alert queue rather than every response. Role-based access gives abstractors, surgeons, nurses and program directors each a view scoped to their role.

Yes. Alert thresholds are configured by the clinical team during implementation and refined afterward. When a patient’s reported symptoms, vital signs or check-in responses cross those thresholds, the care team is alerted — via Epic In Basket where integrated, or email or SMS otherwise — so they can intervene early rather than discovering a problem at an ED presentation. Patients may also receive automated self-escalation guidance advising them to seek care.

Yes. Alerts route by type to the right team member: diet questions to a dietitian, stoma questions to a stoma nurse, mobility alerts to physiotherapy, wound and infection alerts to a specific nursing group, pre-admission screening alerts to pre-admission or day surgery. Alternatively all alerts can route to one person or group who triages from there, and alerts can go to multiple recipients simultaneously.

Yes. Each alert can be assigned a resolution status — in progress, resolved by monitoring team, resolved and escalated to ED, or resolved with no action required — creating an auditable record of clinical follow-up for every flagged event. Aggregate reporting shows which alerts led to intervention versus resolving without it, which is what enables ongoing threshold refinement and guards against alert fatigue.

Real-time dashboards show patient roster, active alerts, PROM completion rates, symptom trends and engagement metrics, filterable by program, surgeon, procedure, site and date range. Longitudinal tracking compares pre-operative and post-operative PROM scores across configurable windows. Complication rate dashboards break down by procedure, surgeon and site for internal benchmarking, and multi-centre PROMs benchmarking between organizations is available. Reports can be scheduled and emailed automatically, exported to CSV, Excel or PDF, or accessed via API for data warehouse integration. Custom ad-hoc reports are also produced by the Customer Success team on request.

SeamlessMD’s standard offering is deviceless remote monitoring — patients self-report readings from their own devices into the app, where those readings are trended and alerted on like any other patient-reported data. This keeps programs lightweight and scalable. However, health systems can provide their own devices to patients, and SeamlessMD can set up custom integrations to those specific devices upon request.
